Eligibility
Primary diagnosis must be medical.
Secondary diagnosis may include mental health or substance use
Independent with Activities of Daily Living
Homeless: Unsheltered/sheltered
Medical: Must be recovering from an acute medical condition although concurrent active chronic medical conditions are allowable.

Ineligible/Contraindications
Actively homicidal or suicidal at time of referral
People with sex offense registration requirements
Absence of medical need
Incontinent

 

Medical Respite Model (National Institute for Medical Respite Care)
Integrated Clinical Care: medical care and mental health services onsite

Common Medical Conditions include but are not limited to:
Pre-op stabilization
Post-operative Care
Dialysis Care Coordination
Cancer
Wound Care
Acute Exacerbation of a Chronic Illness (example - stabilization of chronic diabetes after hospital admission for Diabetic Ketoacidosis)
End of life (only with close coordination with palliative medicine)
