Medical Respite Model (National Institute for Medical Respite Care)
Coordinated Care (offsite health services; connection to community behavioral health)
Common Medical Conditions include but are not limited to:
Recovery from surgery or a recent hospital stay
Wounds that need ongoing care (cuts, ulcers, infections, burns)
Respiratory illnesses (such as pneumonia or other breathing issues)
Chronic health conditions that need stabilization (e.g., Diabetes, Hypertension)
Mobility limitations due to injury or illness (e.g., fractures, weakness after hospitalization)
Infections that require rest and follow-up care
Conditions requiring medication management or monitoring
