Eligibility
Primary diagnosis must be medical.
Homeless: Unsheltered/sheltered
Medical: Must be recovering from an acute medical condition although concurrent active chronic medical conditions are allowable.

Ineligible/Contraindications
Sex offenders
Those convicted of arson
Emotional Support Animals (Service Animals eligible)

 

Medical Respite Model (National Institute for Medical Respite Care)
Coordinated Care (offsite health services; connection to community behavioral health)

Common Medical Conditions include but are not limited to:
Dressing changes
Post operative recovery
Long-term infusion therapy
Recovery from acute conditions like heart attack, pneumonia, cancer treatments

Common Referral Sources or Special Referral Partnerships
Community Health Care

Social Services Staffing
Case worker, Department supervisor oversight; daily check-ins, transportation to appointments, enrollment in mainstream resources (Medicaid, SNAP, etc.)

Housing Processes
Project NOW works will private landlords. Participants are also entered into
- the Quad Cities Bi-State Coordinated Entry By Name list
- the Statewide Referral Network for housing assistance
