About the Network Guide
The Illinois Medical Respite Network Guide is a resource for hospitals, health centers, and community partners to identify Medical Respite programs across Illinois that support people experiencing homelessness during recovery from acute illness or injury. The guide was developed through the Illinois Medical Respite Capacity Building Initiative, convened by Illinois Public Health Institute and made possible through funding by the Illinois Office to Prevent and End Homelessness.
The Network Guide provides a centralized directory of Medical Respite programs across Illinois and includes program contact information, referral resources, websites, and other details to help organizations identify potential referral options. The guide will continue to be updated as new programs become available.
How to Use the Network Guide
Use the filters and search features to find programs that best fit your needs. Programs are currently listed in alphabetical order but can be filtered by:
Status – View programs based on their current operational status.
Location – Find programs by geographic area.
Accessibility – Identify programs based on available accessibility features.
Search – Use the search bar to find programs by keyword or specific criteria.
Model of care – Explore programs based on their approach to providing Medical Respite services. Models of Medical Respite care are reflected in the image below. Learn more about the models of care from the National Health Care for the Homeless Council.
