For nearly a decade, the Alliance for Health Equity has convened to improve population health and promote health equity.
The Alliance’s collaborative hospital-community partnerships and specialized workgroups strengthen community health outcomes in the City of Chicago and Suburban Cook County. Along with completing the area’s Community Health Needs Assessment (CHNA), we convene workgroups to address the social and structural determinants of health and the increasing need for food, housing, and mental health support. Our work has impacted individuals and families all across our service area by connecting local initiatives and leaning on the knowledge of the true experts - the residents of our communities.

Fostering Collaboration
The AHE Mental Health Workgroup is partnering with the Transforming Communities Initiative at Loyola, Cook County Department of Public Health, and NAMI Metro Suburban to host Systems in Sync: Partnering for Equitable Mental Wellness. Systems in Sync is a free event to bring together community organizations, healthcare, health departments, and community members to collaborate, connect, and share best practices to improve mental wellness and reduce inequities in Chicago and Suburban Cook County.

Case Study: Veggie Rx
VeggieRx provides participants with a bag of produce, vouchers - “Produce Perks” - that are redeemable for additional produce at onsite farm stands. Additionally, nutrition lessons and a cooking demonstration using produce that participants purchased with the voucher are available. The cooking demonstration is used to spark discussion on healthy cooking and nutrition.
